Veterinary Referral

The Veterinary Surgeons Act 1966 states that Veterinary physiotherapists are required to assess and treat an animal under veterinary referral.

As an ACPAT registered Veterinary Physiotherapist, I have professional indemnity insurance for physiotherapy treatments. In making this referral, the vet is not responsible for any physiotherapy assessment or treatment given, and the provision of it is the responsibility of the Chartered Physiotherapist. You will be kept updated by a physiotherapy vet report after the initial physiotherapy assessment and treatment Pet insurance companies recognise ACPAT registered Veterinary Physiotherapists.

To refer, please use my online vet referral form or alternatively download the below PDF and return to myself, or alternatively call or send me an email containing the clients details, reason for referral and history.

If you are an owner, and you would like your pet to receive a physiotherapy assessment and treatment, you can print off or email this form to your pet’s registered vet, and request them to give their consent and complete the form before an appointment is confirmed.
